• Provides outstanding customer service
• Maintains HIPAA standards related to Privacy and Security • Provides care coordination and case management services • Provides patients assessment/care, educates patient and the family, and
refers patient to agencies that can assist in meeting health and /or social needs
Minimum Qualifications

• Facilitates physician/patient communication & treatment via Telehealth • Position requires extensive travel throughout the 14 county Health District to include home visits

Associate's degree/diploma in nursing from an accredited college or university and Georgia Registered Professional Nurse license. (Please mail or bring a copy of College Transcript or Diploma to Interview—must have official transcript prior to start date).
Preferred Qualifications
• Experience in providing case management services
• Experience performing physical exams on newborns, toddlers, children and adolescents

This position may require driving company vehicles (Valid driver’s license required) and working some evenings and weekends during special events such as health fairs Southwest Health District 8, Unit 2
and responding to disasters. Selected applicant must consent to a motor vehicle record assessment. Points should be less than 10 and/or not negatively impact insurance rates for the agency. Drug screen, criminal background check, and immunization records may be required. If selected, male applicants between 18 and 26 years of age must present proof of Selective Service Registration.
