POSITION: Transition Specialist

LOCATION: Exceptional Students Program (Albany High Complex)


The Transition Specialist provides special education support and assistance to students, teachers, and staff. The Transition Specialist helps students identify their career path for themselves and directs them to resources that can help them start toward that path as well as suggest scholastic goals and career opportunities to work toward. This individual will ensure compliance procedures are implemented and followed in accordance with Federal and State laws as well as district procedures. This individual will also provide Professional Learning for administrators, teachers, and staff to promote and support the success and achievements of students with disabilities.

QUALIFICATIONS: Bachelor's degree in Special Education; Valid Georgia Certification; A minimum of five (5) years of successful teaching experience in special education; An understanding of the implementation of transition requirements in the secondary teacher role; Such alternatives to the above qualifications as the Board may find appropriate. SKILLS AND ABILITIES: Ability to communicate with staff, administrators, students, teachers, parents, and community partners; Ability to work collaboratively with multi-disciplinary teams to include agency representatives and community partners as part of the coordinated set of activities required in transition services; Ability to apply critical thinking skills and recommendations in rendering solutions to various issues; Ability to exercise strict confidentiality in handling sensitive information.
PERFORMANCE RESPONSIBILITIES: Assists the Special Education administrators to develop, implement, and monitor a district-wide plan to provide appropriate and effective transition services; Acts as the transition contact person for administrators, teachers, parents, and community partners; Attends and collaborates with secondary staff in preparation of the IEP and transition to provide recommendations and support during meetings to determine services for students; Knowledgeable of usage with the Student Information System and GOIEP for data reporting; Conducts informal and formal transition assessments and/or monitors the administration of transition assessments administered by campus staff to identify strengths, needs, preferences, and interests; Counsels and/or works with staff who counsel students with disabilities regarding transition assessment results and postsecondary goals related to education/training, employment, and independent living if appropriate; Effectively utilizes available technology for the development of professional learning activities as well as IEP reviews; Ensures the special education teachers receive support and processes are in alignment with transition requirements; Ensures transition services consider the impact and needs of children with disabilities related to multicultural and diversity issues; Provides professional development and technical assistance on transition service requirements and the appropriate implementation by campus staff; Provide consultation to special education staff regarding transition opportunities for students served by special education as well as information and training for students regarding transition and self-determination; Reviews transition plans and activities to monitor compliance with federal and state regulations; participates in state, district, and other professional learning opportunities to strengthen skill sets needed to assist district departments and schools; Serves as a resource to staff and teachers regarding transition opportunities within the curriculum; Works collaboratively with school counselors and postsecondary institutions to encourage students with disabilities to consider postsecondary education, makes selections based on interests and preferences, and facilitates application activities to the institution and the Office for Students with Disabilities that provide support to students with disabilities in postsecondary education; Works with campus staff and district administrators to provide parent training and parent involvement activities that support transition for their child; Performs other duties as determined by the Supervisor.

REPORTS TO: Director of the Exceptional Students Program

SALARY RANGE: Competitive salary and excellent benefits package in accordance with the DCSS Compensation Guide.
